    Question?

Sometimes my TV/Monitor gets visual streaks running vertically across the screen and I have to turn off my PC and monitor, disconnect monitor from power source, unplug the Super VGA cable from my monitor, plug it back in and turn everything back on to get rid of the lines.

How do I permanently fix this recurring issue?

Sounds like your video card is dying or overheating (or even the monitor it self). The process that you are doing is just allowing either the card or Monitor to cooldown.

Check the fans on your computer (get a vacuum and get rid of that dust around the fans and even on the graphics card itself).

vacuum? I bought compressed air, expensive as hell I might add, I did some serious cleaning but after seeing how it was I need it to be spotless, one idea was taking it all apart but I do not really have the tools for that even though I only need screwdrivers for the most part. And its really late, just got home from work. Tomorrow however I will take it apart and give it a thorough cleaning. There is one thing with the compressed air its whack, I cant hold it for too long its annoying, I need a more effective way to get rid of all the dust.

You can use your natural hot air as well.

Just make sure you aren't spitting as well.

I recommened the vacuum because it's a "pull" instead of a "push" that is compressed air. Plus, it doesn't get the shit all over the place. When you use the compressed air, unless you are doing it outside, that dust will find it way right back in.

I would be Cautious with a vacuum.
As much as I hate dust, static might just fry your device.
But it does sound like the video card is overheating.
Try and test it out with Furmark, if it goes over like 100C theres a problem, try raising the fan speed or something before contemplating on replacing the card.

Which reminds me I have to clean out my PC and clean the dust filters.

I just downloaded Deus Ex HR so it better not be a video card replacement, I highly doubt it is, ive had this one for about two years maybe the GTX 260. Its suitable for the task.
